Be sure to fill out the application clearly and completely. You can type directly into the form or print it and complete it by hand.

2. Print Your Completed Application

Once you’ve filled it out, print the application and sign it where required.

3. Bring It to a Chapter Meeting

Please bring your completed application in person to our next chapter meeting, along with your membership fee.
